Question
I tried to send a legitimate second chance offer to the 1st underbidder. When I clicked on the link, it brought up a list of the 2rd, 3rd, 4th and 5th underbidder, not the 1st. These bidder had bid way under, so there's no sense for me to send it to any of them.
I have no way to send it to the 1st underbidder since it does not present it on the underbidder list.

It may just be that the first bidder has on their preferences that they will not accept second chance offers.
I have run accross this before and that was the reason.

Yep, That's what it is. On the page for your second chance offer it mentions the list is for those who have settings that will accept SCO's.
Sadly, I've had the same thing happen when wanting to make a SCO and the list for the one who would has bid too low to make it worth the offer.

Is the 2nd chance offer default not to accept? Why wouldn't someone want to buy the same item cheaper?

The default is to accept getting the offer. You have to go in and turn that off if you do not want to receive them. I have probably made 3 or 4 offers and none have been accepted. Go figure.
