I am in Ohio
after recieving STD they decided to review and denied me STD & LTD.
Reasons given are my medical records do not Quantify my injury. They say they know I am injured but that they need medical evidence.
I have a severe Lumbar back strain.
There is no Tests that can show the injury. Is that what they are saying?

If the STD policy says they have to have medical evidence (which most do), then they have to have medical evidence. This is not a question of law; it's a question of what the insurance policy says. The law does not dictate that medical evidence is required; nor does it prohibit the carrier from requiring it.
It is not unreasonable for the carrier to want medical evidence before making payments.
