I recently lost my husband, and while going through all of his paperwork I found that on one of his life insurance policies still has his ex-wife named as the beneficiary. This was an obvious oversight considering they had didn't have a very good relationship. Is she still entitled to the policy?

Yes she is. And unless you commit fraud then you can not cash in the Policy.
