Does anyone have a copy of a Chairman's Card Application that they can pm, email or fax to me? I really need to find a copy as soon as possible. Any help would be appreciated. Even one that has to be whited out is ok. Thanks.
I have been on the phone with citigroup about this problem and they have told me to contact any source that might have one. I have tried the Financial Advisors and now I am turning to the only group that I know of that would have anyone that might have stored one or kept a copy of one. Surely there is a FT'er that has one of these in a file somewhere? I am asking for your help. Thanks in advance.
