The United / Chase does not earn "Choices" on balance transfers. From the Chase / FirstUSA terms and conditions:
"Choices or miles are earned on net purchases only. Choices or miles are not earned on balance transfers, convenience checks, cash advances, or fees, including fees for products that protect or insure the balance of the cardmember's account. Bonus Choices or miles do not count towards caps."
I do not know of any card that currently offers miles for balance transfers or convenience checks.

Juniper US Airways (up to 10k), Juniper Midwest Airlines (up to 10k). I believe the BofA US Airways card may have something similar, but I'm not sure.

Note that cards that offer miles for transfers also charge a fee of up to 3%, so it may not be worth it.
