Special offers from MBNA (soon to be BofA) or your Credit Union offer low fixed rates. I have a fixed 4.9% rate from my CU with no foreign exchange fees, and an MBNA MC at 5.90% fixed. Both were special offers mailed to me.
Keep in mind the fixed rates can change at anytime if the bank or cu notifies you in advance.
For 0% Balance Transfers try Citibank, Chase, or look for possible flyers in larger local Sunday newspapers with 0% offers.

There are many variations on this card, but all require some monthly purchase after an introductory period. If you're fortunate and have no monthly purchase minimum and a relatively large credit limit, this card pays quite well!
Edit: Rate mentioned is for balance transfers. It's not clear whether you're looking for a purchase or balance transfer rate.

I think that Discover Platinum Card has 0% lifetime interest.
But here's something more important:
Many companies will attract applicators by stating that
they're 0%.
But when you get their actual application where you fill in the
details, you'll see at the bottom (in microscopic writing) that
this only applies to 12 or 24 months.
Or it expires after 12 months but then you have a choice to
prolong it for life only if you loose some other benefit that
the card initially offered.

I have the old Citibank Illumina Black MasterCard with 3.99% Fixed APR for purchases/balance transfers. However cash advances I believe is 15%. CapitalOne also offers a 4.99% Fixed APR account. These are the lowest I am aware of.
If you're looking for balance transfers, I just got an offer for 2.9% Fixed for the Life of the Balance through Citibank. Try calling them and asking about this offer.
