Cards that give 5% cash

Too many offers! Too much information!
I am trying to narrow down the best offer for a CC giving at least 5% on some purchases. The credit cards I know of include.....
Chase
Citi
Discover

Please list the offer information you find on any credit card offering at least 5%. Hopefully we can compare apples to apples and have a similar information to compare each deal if everyone uses the same format for posting.
I found this offer online for Chase which sounds great... has anyone tried it out. Does it really work? Please note: I cannot verify that the number listed to call or the offer code is valid.

I have come to know about the Discover offer quite well, so I will try to start with the details on that.

I've had this card for two years and have received the maximum $600 in rebates ($300/year). Rebates can be requested online and come with no hassle.
I've been maxing out on rebates with the Citibank card by summer, so I just applied for the Chase card (Thanks for the info on the $100 bonus, ClassiCair!) and will use it the remainder of the year.

You are welcome! Yeah, I thought that was a pretty great deal.... better than the Citi card for the bonus. I just found a similar ad in my Sunday paper this weekend with the same phone number but only offering $50. (I was a bit concerned that anyone could create a fake flier for the card, post it online with a fake phone number ,and then steal everyone's SSN etc while they applied for the card over the phone.) This is not the case however. Phew. (assuming you trust me too, if not, check the manufacturer coupon section in your paper for a similar offer from Chase)
From what I have read on FatWallet.com, the code for $100 doesn't always work. There are three codes I have heard of which generally work for the $100 cash back. Those are HG1, Z3R, HF9. You have to call to apply for the card (usually to a call center in India) and make sure they verbally state that you will get the $100 reward before actually signing up. Sometimes it sounds like they will say that it is not available, and you only qualify for $50. If they say that hang up, and try with a different rep. Keep calling giving the different codes until someone will verify that you will get $100 and then finish the application process. Someone on fatwallet also suggested that you don't give them any information about yourself (ie your location) until they confirm the $100 reward. Deadline: Oct 31st.

Also, American Express Blue Cash offers 5% on gas, supermarkets, drugstores AND HOME IMPROVEMENT. So that is a great card as well, but since it doesn't offer the $100 signing bonus, I think the chase card is better. But if anyone maxes out your rewards on Chase, the AmEx may be a good opiton.

American Express Blue Cash requires that you charge $6,500.01 before you will earn the 5% in there "Everyday Purchases". And from $0-$6500.00 you only earn .5% on all purchases and 1% in "Everyday Purchases" thus if you spend $10K a year all lets say in there so called "Everyday Purchases" your cash back would be $240 vs $500 With Citibank and Chases. Ofcoarse both Citibank and Chases have $300 cashback limit so after $6000 dollars charged you want to switch to another card.
If you goto Fatwallet.com read there cash back thread there are a couple of VISA/MC that pay flat 1.5% cash back on anything you buy like the fidelity cash back card. Requires you have a fidelity brokerage account but still pays 1.5% on all purchases.

Just to let everyone know, Cash Plus is a rewards program from chase and can be added to a variety of their cards. You don't specifically have to get the cash Plus card. For instance, I have the World Wildlife Fund (WWF) card which donates 1% to WWF on all of my purchases and also have cash plus on this card. Therefore, I look at it as if I am getting 2% back on my regular purchases on 6% back on my everyday purchases since I am essentially making a donation to charity as well. They offer this on their St. Judes Card as well. Some of the other cards don't give a full 1% back to charity and can be as low as 0.5%. But still in my opinion, if you don't plan on carrying a balance on the card it is worth it to take them up on this option because you can get all of the same benefits as with the regular cash plus card and also be helping a charity that like. (I must note, however, that you won't see this advertised on Chase's website. The reason is that it would encourage more people to get these kinds of cards and cost them more money in the long run.)

Citi is offering Citi Simplicity Card (http://citi.bridgetrack.com/usc/05/simplicity/Default.htm) which 3 reward options; lower rate, ThankYou Points or cash rebates. If you look at the ThankYou Points and cash rebate options, 5% rebate (or 5 pt/$) is only good at gas and drug store charges. Grocery charges are droped to 1%.
I guess, 5% rebates are very generous and it won't be lasting long. Anyway, I'm gong to take full advantage while it lasts!
