What usually happens is that the miles stay, but you will not receive miles for the first $2000 you spend in the next billing cycle. I am in this sort of position with my Delta Skymiles card right now. I happened to have returned a $1000 item after the miles posted. The miles remain in my account but the next $1000 I spend will earn me nothing. So I've had little incentive to use the card lately!

I did have a miles adjustment on my UA MP platinum due to a reversed charge. Not sure how the transaction was processed thru MP -- if reversals are different than exchanges. Charged a $3,300 dentist bill -- dentist inadvertantly submitted it to credit card company as $33,000! Now that was some dental work!
Took about 2 weeks to get it straightened out across a billing cycle. Got nice miles and then the next month watched those miles disappear. :(

My MP account was deducted by 2 miles due to a chargeback last Jan. I think they decided to take it away directly because it's such a small number.
If it's a chargeback, I believe they won't deposit more miles until you make up the difference. In your example, it would be another $2000 new charges before you get new miles. Unless you are able to request them to issue you a check for credit balance. I've never done this with Bank One. My NW visa (US Bank) had issued me credit checks twice upon my request when I realized I mindlessly overpaid my balance. Don't ask me how could it happen :p

What I often do in situations like this, when I am returning merchandise, is have the merchant post the credit on a different VISA or M/C - one that is not a miles earning card. When I get a credit balance on this card, the card issuer will deposit the credit balance into a checking accoun, or mail me the credit. That way, I don't lose the miles I earned with the original purchase.
