Is there a way to find out what code the store you plan on shopping is to see if you will receive the 10% credit? Can you collect $25.00 for each of the month until the expiration of the offer?

Just read through some of the fatwallet thread on this. Sounds like if you use code HGW1, the 10% applies to all purchases. And you can get multiple cards to get around the $25 rebate limit.
