This isn't miles-related, but it will drastically cut costs for those who travel overseas a lot.
Six large banks have formed a Global ATM Alliance (http://www.scotiabank.com/cda/content/0,1608,CID4710_LIDen,00.html) allowing customers to get cash throughout the world without paying ATM charges.
Simply get an account with one of the banks and then use an alliance member's ATM while travelling.
