Mileage debit card?

Question
For various reasons, I like to use debit cards instead of credit cards (no bills, no annual fees, linked to bank account with billpay (usually) so I can see all the outflows in the same place, segregation of my money and company money, etc.). I would like to be able to get a debit card that awards miles.
I signed up for a Compubank account a while ago when they were still awarding miles but then they quit. Now I'm looking again.
Suggestions?

Chase offers a Continental debit card.
I heard that Citibank offers a AAdvantage debit card, though I haven't seen it.
NationsBank has a USAirways debit card.
All require that you have a checking account with that bank. They all have different quirks, but basically all have an annual fee and award miles only for offline (non-PIN) transactions.

Not such a good deal.
I wasn't able to find the Citibank AAdvantage debit card, but each of the others you mention award one mile for two dollars spent and have a $30 or so annual fee.
I guess I'll stick with plastic.
