Few months ago, the Northwest Airlines WorldPerks Visa Card (issued by US Bank) sent out a form for customers to sign up for the AutoPay and earn bonus WorldPerks miles. Does anybody still keep the AuotoPay sign-up form and be available to mail it to me?

I just calling them to get the form with the bonus miles and they said that you couldn't request it and that they are automatically selected by computer to be sent out to people.
So, if anybody else has any extra after my fellow colleages above are fulfilled, by all means, please let me know. Thanks!

I called them a few days ago and told them I had lost my form and asked them to mail me a new one. They said that there was a promotion that had already ended, but that there would be a new one starting at the end of September and that they would gladly send me the form. (3000 bonus miles)
