I know that I demand stupendous detail in my posts, but this time I really need it:
Please describe the exact insurance types and requirements I will need if:
1) I own a tractor and trailer, have my own authority, I will drive myself, and WILL NOT haul haz materials.
2) I lease a tractor and trailer, have my own authority, I will drive the truck, and WILL NOT haul hazmats.
3) I will rent a tractor and trailer, have my own authority, I will do the driving, and WILL NOT haul hazmats.
4 I Will lease a tractor, tow a COMPANY trailer, have my own authority, I will drive, and will not haul hazmats.
5) I will rent a tractor, tow a company trailer, have my own authority, I will drive, and will not haul hazmats.

I will attempt to provide you with some GENERAL answers to your scenario's. Keep in mind, that "need" and "required" may not be the same thing.
If I were looking at your particular situation, I would start with the following. Since each situation is different, based on all the particulars of your operation (which would be difficult to discuss in a post) other coverages and endorsements may be necessary.
1. Physical Damage (if you have a lien on the equipment, Liability, Cargo, General Liability, Occupational Accident or Workers Compensation.
2. Physical Damage with Additional Insured Lessor Endorsement, Liability, Cargo, General Liability, Occupational Accident or Workers Compensation.
3. Same as #2
4. Physical Damage with Additional Insured Lessor Endorsement, Hired Car Physical Damage, Liability, Cargo, General Liability, Occupational Accident or Workers Compensation.
5. Same as #4
