I need get a physical before going back to work, it expires before I will be able to return to work (I broke my wrist). The company has offered to take care of it because they want their Doctor to release me for my wrist also. I would rather have my physical and release in hand, that way if their doctor should refuse me I will be able to move on quickly. I don't really anticipate any problems but a physical in hand is a security I would prefer.
ANYWAY....... Where can I get a copy of the DOT physical for my Doctor, his nurse said I need to bring it woth me.

Why would you pay for your re-ceritification if your company is going to do it? 3 years ago I had injure my hand, went through COMP Dr. for treament and theraphy. Did not have any problems being re-certified after my hand was healed.

Are you requesting how to get the long form in order to take to your doctor?
If so it is pretty standard practice for doctor offices to have these forms, or know how to make them available. Health department might be able to supply you with blank form or even be able to do the physical for you with little charge.
If you're talking about your previous physical long form: your company is supposed to supply you with a copy of theirs (provided they did one) and like the card- it is supposed to be available at all times when on a CMV. I always kept the long form physical on the bottom inside of my log/ BOL clipboard--just incase I had to produce it, along with other certifications proving I met certain additional training per new Federal guidelines.Rejoice in the LORD
