Answer
I've suffered from migraines since the early 1990s. The doctor gave me Imatrex (nasal spray) to use when a migraine was coming on. Instead of being laid up for hours (or days), I'd be back on my feet in 20-30 minutes. At this point, I was having them pretty much daily. As the cycle began to be broken, they became much more infrequent and now it's once or twice a year. When driving, I'd naturally stop when one was coming on and wait until it was completely passed before leaving out again - usually creating about an hour delay. The Imatrex doesn't make you drowsy or have any of the effects that Topamax has.
I looked Topamax up on WebMD.com and it looks like it's pretty strong stuff with a lot of side effects. I think I'd get a second opinion before starting it.

After all of these years I still get migraines. The best cure I have found is 1600mg of Ibuprofin. Yes it is a lot, but it works every time. Take the pills, go lay down for 30 minutes and I'm good to go. You may also want to try pinching the skin between your thumb and index finger when you feel one coming on, sometimes that works as well. For a "normal" headache that will cure it every time.

I doubt you can still drive on that med. It is also used as a anticonvulsant.
All meds have side effects but these are a bit different.
