Question
I have a severe hearing loss in my left ear and a slight high frequency loss in my right ear. Since my last DOT physical nearly two years ago, I know I have lost more hearing in the right ear.
The problem is that three times in the last year, I was almost hit (one time I almost hit them) by a fire truck coming thru an intersection. I heard the siren, but with my hearing loss I have lost my sense of direction. I had (and still have) no idea where an emergency vehicle is coming from.
Do I tell the DOT Dr. who is going to do my physical about this?
I really need some advice. Any and all comments welcome.

Seems to me you may be putting lives in danger driving in that condition. Perhaps a hearing aide would help the situation. I would ask your doctor. When was the last time you had a hearing test?Marines stay and fight, Bush makes sure of that.

The DOT standards regarding hearing are pretty minimal. You only have to "perceive a forced whisper" at 5 feet or greater, in the better ear. Doesn't say anything about understanding the words (a much harder test). The intent is that you hear warning horns, sirens, etc.
However, as you pointed out, with functional hearing only in one ear your ability to determine sound direction is greatly reduced. I think the decision as to whether or not you can operate safely is left to you. Of course the physician will fail you if you want, but they can also pass you under the part 391.41 standards! I would just sit still until you see the lights if you aren't sure where the siren is coming from..
A hearing aid is a good idea if you have enough hearing in that ear to make it worthwhile...
