I need to get my med cert card renewed. The doctors office doesn't have the long form, and the place I work for doesn't have them. Help! where can I pick one up asap? Appointment for this is monday.

Your local MVA should have what you need.
I would offer to fax your physician one, but I ours are pre-printed with our information.
My main concern is that you physician doesn't have these forms. If he doesn't have them how does he know what the regulations are for a CMV operator to pass medical certification? Some of the requirements for a DOT physical are different than what your physician may consider 'normal' in any other physical exam. For instance, a blood pressure of 158/92 is not going to excite your Dr. to the point where he would put you on treatment right away. But this blood pressure will not pass a DOT exam.
Your physician may not be concerned about your inability to distinguish colors, but the DOT will be. See what I mean?
I suggest that you look in the yellow pages and find a clinic that specifically does DOT physicals.
