Is the new dot physical form a total of 4 pages, that is actually 2 pieces of paper with writing on both sides with page 4 having a big square box that says Medical Examiners Certificate? This is the one mr Dr. filled out that I recieved from Ft. Scoot CC traing school. Is this the same type used by the trucking companys?

8 page version of the form being marketed by J.J. Keller. They may do that provided that all of the required components of the exam are on whatever form they are using.
So I have no idea which one you may be looking at. But it sound like it may be the same.
Make lots of copies and take it with you to whatever Dr. the company sends you to.
