Where can I find the 8 page instructional form for doctors that you referred to? Is it available online? Does it have an identifying number?

I'm not Nurse Red but the regulations are all in the FMC Safety Regulations handbook section 391.43 Also if you are going to work for a carrier they will send you to their physcian to get a physcial.
I know that I went to a doctor for my physcial at one company and then after orientation realized that i had made a big mistake went to another company 1 week later and have to have another physcial even though I had my long form..It may save you some money.

I printed out the section referred to by Little Red (Thanks for the info, Little Red ) and it came to 8 pages. Was that a coincidence, or was that section (391.43) the instructions you meant?
