Can any doctor give a DOT physical? I'm going to see a VA doctor soon. If I give him a physical long form from a truckstop and if he fills it out properly, will that qualify as a DOT physical?
I'm not concerned with any carrier accepting the physical, just with whether the DOT or other law enforcement group will accept it.

That was designed specifically for physicians who are performing a DOT medical exam but are NOT familiar with the regulations. I suggest you obtain one of these forms and take it with you when you visit your Dr. as he/she is not likely to have one on hand.
As long as the exam is done in compliance with the regulations and the accepted form is completed correctly, there should be no problems. Make sure when you get one of these forms you also get a DOT card with it.

You may have to order them in quantity. That's the only problem with that option. You may try calling a local Occupational health clinic in your area, but they may not sell you one.
