Does anyone know where I can get blank copies of the Medical Form (Long) and the Wallet Card. I searched the DOT and FMCSA web sites and couldn't find them under the "forms and reg" areas. Maybe I just odn;t know where to look!

Almost any truck stop will have them. Look in the area where you find log books and bill's of lading.
But .. either your company or the Dr. doing the physical "should" provide them.

Yea, it does sound kind of weird - DOT/DOD, they should know about each other...
But, most states have an exemption for military vehicles - you don't need a CDL to drive them (as long as you're in the active military.
There's lots of people on this board who have gotten lots of experience driving trucks in the military and then have to go get a CDL when they get out (and are looking for a job).
But you're right - it does sound goofy!

During the past month, I renewed my CDL, took my company physical thorugh Miller Transporters to renew my DOT medical card and renewed by DOD heavy truck/equipement license. What is interesting, is that the physical requirements for the government tractor-trailer license are some what different than the DOT physical. For example, blood pressure readings as high as 160/100 are acceptable versus 160/90 on the DOT requirement.
