-On the second day the kne was better and on the third day it had completely dissappeared.
-She had no shaking chills, but her husband said she was very hot in bed last night.
-This patient has been depressed ever since she began seeing me in 1983.
-Patient was released to outpatient department without dressing.
-The patient will need disposition, and therefore we will get Mr. Blank to dispose of him.
-Discharge status - Alive without permission.
-The patient refused an autopsy
-The parient has no past history of suicides.
-The patient expired on the floor uneventfully.
-The patient's past medical history has been remarkably insignificant with only a 40-pound weight gain in the past three years.

i dunno, doctors are really busy people. some of them are kinda stupid like the suicide one, but others kinda seem normal. 'patient was released to outpatient dept. without dressing' maybe they're referring to wound dressings?
