Questions on medical insurance

1. Deductibles:
Is the deductible amount applicable during the very first doctor visit? Or do we pay the deductible for each dr visit?
2. Co-insurance/Co-pay: For Protection America, the copay is 20%. Does that mean the patient always pays $1000 outof pocket plus the deductible for each doctor visit? That means when the doctor bill is less than $1000+deductible, the patient always has to pay the complete amount?
3. Is there any reduction in cost if I take the insurance one time for 6 months as opposed to renewing it one month at a time?
4. I see a $40 difference per month when I did a quote for my parents on Patriot America and Protection America with $250 deductible. Can you explain why. (What additional benefits does Protection America have)

1. Deductible is annual.
2. 20% for co-insurance for first $5,000 in medical expenses is just once. You continue to pay 20% every time until you pay total $1,000 out of your pocket.
3. No. Whenever you renew the insurance, insurance company charges $5 renewal fee. You have save that by buying for 6 months.

This means we pay $1000 out of pocket only once as long as my policy is active, correct?

Generally, yes.
If you keep the policy for more than 1 year, you have to pay the deductible and co-insurance again for the next year.
