Answer
Both are fixed coverage plans.
Definitely choose Inbound USA.
Reasons:
* If you look at the policy wording of ICICI Lombard travel insurance (NOT the brochure), dental section has 9 exclusions.

Among them,
2. Any pre-existing condition.
3. Treatment, which could reasonably be delayed until the Insured's return to the Republic of India. ........
And 7 other exclusions.
Now, in dental, pretty much everything is pre-existing. Tooth decay does not happen overnight.
Therefore, the actual coverage and perceived coverage are totally different.
* ICICI is an Indian insurance company. And you have to call people in New Delhi every time before going to the doctor. Now, honestly, which doctor/hospital would be willing to deal with claims people in New Delhi? That means, in most cases, you would end up paying up front and claiming from them.
On the other hand, Inbound USA is from SRI, a company located in Carmel, Indiana, USA. You get the health insurance card and most doctor/hospitals would bill insurance company directly. Read more about it at http://www.buyamericaninsurance.com/...sa/network.jsp
* Read all the advantages of buying from US company at http://www.buyamericaninsurance.com/guide/index.jsp
* And as far as real dental coverage, look at Careington plan at http://www.buyamericaninsurance.com/...dental/all.jsp
It is only $11.95/month per family (both mother/father included) and there is one time application fee of $20.00 It works great. No claims to file. Just take the card to the participating dentist. I have used it personally for my visiting relatives and it was very smooth.
In short, buy Inbound USA for medical and Careington for dental.
And to answer your other 2 questions:
1. Pre-existing conditions are any medical conditions that the person has before the effective date of the policy. They are not covered in any plan from any company, no matter who is promising what.
2. No, the proof of insurance is not checked at the port of entry by immigration officers in US.
